Overview

Concussion and mild traumatic brain injury (mTBI), used synonymously in the literature, have received enormous attention in recent years, both in the popular press and the scientific literature, especially those related to hockey and football. The Centers for Disease Control has estimated that 1.6 to 3.8 million cases of sport-related concussions occur annually in the United States. Sport and recreational activities contribute to about 21% of all traumatic brain injuries among children in the US.

Concussions are caused by a direct blow to the head or other body part resulting in a rotational movement of the brain within the skull. It is important to recognize that a concussion can occur with or without loss of consciousness and symptoms can be subtle, including headache, confusion, nausea or dizziness, and may not appear for hours or days. Recommended treatment includes both physical and mental rest. If an individual returns to activity too soon and a second concussion is sustained before recovering from the first, a condition known as second-impact syndrome may occur: a swelling of the brain that can result in brain damage causing severe disability or even death. Furthermore, an individual is three times more likely to sustain a second concussion during recovery from a concussion.

Dr Babul's specializes in concussions/traumatic brain injuries and their prevention, recognition, treatment and management. She also examines trends and patterns of concussions in BC; and explores knowledge translation activities to raise awareness of concussion and mTBI among targeted audiences, including medical professionals; parents, coaches, and athletes; as well as school professionals. She is the chair and/or member of several provincial and national concussion advisory committees including the Chair of the BC Concussion Advisory Network, and is also a co-principal investigator on the Active & Safe grant-funded project, and co-investigator on the concussion-related SHRED (Surveillance in High Schools to Reduce Injuries) NFL grant-funded project.

Concussion Awareness Training Tool - CATT
The Concussion Awareness Training Tool (CATT) (http://cattonline.com/) is a series of online educational modules and resources with the goal of standardizing concussion recognition, diagnosis, treatment, and management. Good concussion management may decrease the risk of brain damage and potentially reduce long-term health issues.

Developed by Dr. Shelina Babul, CATT is based upon the established principles of the Consensus Statement on Concussion in Sport. Research and evidence on concussions is evolving and the knowledge base is continually changing. As a result, cattonline.com is updated on a regular basis to provide current information, tools, and resources to support concussion recognition, diagnosis, treatment, and management.

There are 7 CATT online educational modules, and all are available free-of-charge.
• CATT for Medical Professionals
• CATT for Coaches
• CATT for Parents
• CATT for School Professionals
• CATT for Athletes
• CATT for Workers and Workplaces
• CATT for Women’s Support Worker

Work is currently in progress for the development and evaluation of CATT for Youth, aimed at high school students.

SHRed Concussions
Youth account for more than 50% of the annual burden of over 3 million concussions in North America. While not all concussions are sports-related, children/youth who engage in sports are at a higher risk for sustaining a concussion.

Researchers at BCIRPU, UBC, and UBCO are conducting a study to improve recognition and management of sports-related concussion in children/youth. Surveillance in High Schools to Reduce Concussions and Consequences of Concussions (SHRed Concussions) is a three-year longitudinal, Canada-wide, cohort study aimed at reducing the burden of sport-related concussions (SRC) and their consequences across youth sport populations. Researchers are recruiting high school athletes who participate in school or club sports associated with a higher risk of concussion; specifically: basketball, football, ice hockey, ringette, lacrosse, rugby, soccer, volleyball, cheerleading, wrestling, alpine skiing, or sledge hockey).

Participants will be asked to complete various assessments over the course of the study, and any participant who sustains a concussion will have access to accelerated concussion care by being immediately scheduled to see a sports concussion specialist. Recognizing and treating symptoms as soon as possible will allow for faster and safer return to school and sports.

This study is being led by the University of Calgary and is funded by the National Football League. Researchers at BCIRPU are coordinating and implementing SHRed in Vancouver. To get involved, visit https://www.injuryresearch.bc.ca/shred-concussions-study/.

The unintended consequences of implementing stay-at-home policies during the COVID-19 pandemic: Violence outcomes in children and youth in Canada
Public health measures aimed at protecting Canadians from the spread of COVID-19 appears to have placed children and youth in particularly vulnerable situations. In the early days of the pandemic, Kids Help Phone calls in Vancouver increased drastically, as did as the number of reported domestic incidents, sexual assaults, and a rise in reported gun violence in Toronto. The Raising Canada 2020 Report states that children and youth and low socioeconomic groups have suffered disproportionately as a result of COVID-19 policies; the number one threat being preventable injury.

With funding from the Canadian Institutes of Health Research COVID-19 Initiative, researchers at the BC Injury Research and Prevention Unit, (a partnership of BC Children’s Hospital Research Institute, PHSA and University of British Columbia), together with other investigators from British Columbia and Ontario, will be collaborating to investigate violence-related injuries among children and youth during the COVID-19 pandemic. The objective of this research is to evaluate the extent to which each phase of the COVID-19 stay-at-home and physical distancing policies affected rates of violence-related injury hospital visits, compared to the pre-pandemic period in both BC and ON. The study will also examine how COVID-19 policies affected violence-related injuries in different neighbourhoods, and whether the delivery of universal infant injury prevention program in BC (Period of PURPLE Crying Program) was more or less effective at preventing violence-related injuries among children than in ON, that does not have a universal program.

‘Active & Safe Central’: development of an online resource for the prevention of injury in sport and recreational activity
The objective of this Active & Safe was to use an integrated knowledge translation approach to develop an end user-driven digital platform that provides injury prevention information and resources across 51 sport and recreational activities. The ‘Active & Safe Central’ (https://activesafe.ca/) platform provides web-based sport injury and prevention information. This user-friendly, web and mobile accessible platform can increase the reach, awareness and implementation of prevention programming in sport and recreational activity.
