Overview

My research is focused on improving health care and patient well-being in children with congenital and acquired heart disease. I am especially interested in the long-term follow-up of patients and how different factors influence their overall health and longevity. A specific interest of mine is how physical activity and exercise affect health outcomes and how we can use traditional and non-traditional methods to rehabilitate children with prolonged illness or following surgery. I am also interested in looking at environmental strategies for reducing greenhouse gas emissions and costs associated with healthcare travel. My colleagues and I are also using digital health tools to enhance communication between patients and their healthcare providers, improve medication adherence, and facilitate lifestyle behavioural change.

Did virtual health visits play a significant role in reducing travel-related greenhouse gas emissions during the COVID-19 pandemic? A comparison with travel to in-person visits to BC Children’s Hospital and community outreach clinics
This project examines the impact of virtual health visits (telehealth) on reducing travel-related greenhouse gas (GHG) emissions for pediatric patients seeking specialized cardiac services. As we reflect on all of the implications that the COVID-19 pandemic played on our personal health and that of our health care system in general, one of the strategic changes that occurred as a result of limited in-person visits to medical clinics and hospitals was a large increase in the number of virtual health visits offered by physicians to their patients. This was done to mitigate the risk of transmitting the virus, but there were a number of other benefits that resulted, including a reduction in the time to schedule an appointment, less time away from school or work, not having to worry about travelling long distances to and from the appointment and potentially lower costs associated with the travel. It is important to recognize that not all medical appointments can be held virtually and some medical conditions require in-person visits for diagnosis, specialized medical imaging and treatment. In addition, some rural areas may not have access to high-speed internet or may not have any internet at all. With this in-mind we seek to determine the role of virtual health visits in reducing GHG emissions and whether or not the pandemic played a moderating role.

Virtual reality and active gameplay as a model for providing physical activity in children with congenital heart disease
Recently, virtual reality (VR), an artificial virtual environment that is created with software and presented in a way that is experienced similar to the real world, has been recognized and used as a viable approach to promote physiological, psychological and rehabilitative outcomes in both private and public health settings. In fact, VR-based rehabilitation programs have been found to be superior to traditional rehabilitation in some situations. Although the use of VR is promising, its use in pediatrics is extremely limited in scope. When used for therapeutic purposes, previous reviews have suggested that VR-based technologies provide distinct advantages when compared to traditional programs. Firstly, VR is perceived as more exciting and motivating than traditional programs which can improve adherence to otherwise repetitive and boring exercises. Secondly, VR can present any scenario in the virtual environment, allowing patients participating in the program to develop outcomes by performing similar, if not exact, behaviors that build toward the desired abilities in a safe and controlled environment. Lastly, the VR headset can eliminate distractions, allowing the patient’s full cognitive attention to be focused on the practice behaviors. Distractions can progressively be added to simulate additional cognitive load and stressors expected to present in the real world. In addition, VR could create exercise scenarios that are effective in improving cardiorespiratory fitness and strength, while improving QoL in patients with CHD. The goal of this project is to build on our previous experience with in-person, home-based exercise and live-streamed exercise to encourage and engage children with CHD to be physically active.

Developing an electronic patient portal for modifying lifestyle behaviors in adolescent cardiac patients
Adolescence can be a vulnerable period of time for our youth. From a health perspective, approximately 18% of all adolescents in BC live with a chronic health condition. In addition to medication and treatment, lifestyle choices including diet and exercise can have a large impact on their physical and emotional well-being. Accessing and implementing lifestyle interventions provided only at specialized clinics at BC Children’s Hospital for these children and their families can be costly in terms of lost wages and travel expenses and time-consuming in terms of absence from work or school. These issues may impact the quality of care that patients receive and is neither equitable nor inclusive and may discriminate against families who live in remote parts of the Province or are socio-economically disadvantaged. Digital health interventions can offer affordable and scalable methods to support healthy lifestyle choices, especially in a province the size of B.C where 82% of children aged 0-19 live outside the lower mainland. For the past five years our group has used digital healthcare tools to support and change how we deliver care to our adolescent patients. We have launched a Short Message Service (SMS, text messaging service) to improve medication adherence in solid organ transplant patients and quality of life in patients with dysautonomia of adolescence. We have also developed and implemented a virtual exercise program and a virtual reality exercise program with our adolescent transplant patients. As we attempt to facilitate lifestyle behavior change, we are now employing coaching strategies and need a digital tool that will allow patients access to educational materials and care plans personalized for their individual health and well-being. One option we are exploring is to use an electronic patient portal. An electronic patient portal is a secure website that allows patients to access educational information, review test results and treatment plans, allows patients to communicate directly with their Health Care Provider (HCP), and it can be linked to an Institutional electronic health care record.
