The aim of our research at the Vaccine Evaluation Center (VEC) is to lower the burden of childhood infectious disease through vaccination by building an evidence base for local, national and international vaccine policy. My research program includes laboratory studies to develop new and better vaccines, clinical trials to identify how best to utilize new and approved vaccines, and population-based investigations to identify targets for new vaccines and demonstrate vaccine impact.

Better protection against penumococcal and meningococcal infections
S. pneumoniae and N. meningitidis are primary causes of septicemia and meningitis. In Canada and other developed countries, case fatality rates for invasive disease caused by S. pneumoniae and N. meningitidis are 5-10%. Long-term disabling sequelae (e.g., deafness, epilepsy, motor impairment, and blindness) occur in one-fifth of bacterial meningitis survivors, most often after pneumococcal meningitis. In BC and across Canada, routine childhood immunization schedules include vaccines against all of these pathogens, yet they still cause a significant burden of serious disease. Vaccine introduction also exerts selection pressure on the target bacteria, modifying the genetic epidemiology and disease transmission. There is therefore a need for ongoing evaluation of existing vaccine schedules to ensure they provide optimal protection, and also to better understand host immune responses and bacterial evolution following vaccine introduction. I am currently undertaking randomized controlled trials to evaluate existing pneumococcal and meningococcal vaccine programs in Canada, as well a study of the molecular epidemiology of S. pneumoniae in children with pneumococcal pneumonia.

Development of a vaccine against Haemophilus influenzae type a
Haemophilus influenzae type a (Hia) causes meningitis and septicemia, with particularly high rates in indigenous populations. In this collaboration with the Northern Ontario School of Medicine, we will be assessing specific risk factors for Hia disease, the burden of invasive disease, mucosal disease (such as otitis media and pneumonia) and asymptomatic carriage of Hia. We will also be carrying out seroeipdemiology studies to assess anti-Hia antibodies in salvia and serum as an indication of exposure. These data will provide the information needed for the implementation of a new immunization policy for a Hia vaccine which is currently in development. Determination of immunological correlates of protection against invasive Hia disease and identification of the target populations groups for immunization will be of particular importance to design future clinical trials of these vaccines.
